FUNCTIONAL CAPACITY EVALUATION (FCE) INSTRUCTIONS
A Functional Capacity Evaluation (FCE) is an evaluation of a worker's safe ability
to work from a physical ability and motivational
perspective, with consideration given to their medical impairments and pain tolerance. The FCE helps determine the
injured worker's ability to return to work safely on their current job, a different job with their existing employer or a
new job with a new employer. The FCE can be a significant event that helps the injured worker and their
family return to a normal lifestyle
again.
If your Knee is injured, wear or bring loose shorts that will pull up to your Groin
so your Upper Thigh can be measured.
Most FCE’s take 1 to 2 hours to complete, although some may last as much as 4 hours, so please wear loose, Casual
clothing, or “Work-Type" Clothing, such as, Jeans, Work Slacks, Tennis Shoes or Work Boots. Please
Do Not Wear Dress
Clothes, such as, Skirts, Dresses, Dress Slacks and Dress Shoes.
PERMANENT PARTIAL DISABILITY (PPD) EVALUATION INSTRUCTIONS
A Permanent Partial Disability (PPD) evaluation, or an Impairment Rating Evaluation, is an evaluation of an injured
worker's loss of function of a body part following a
job-related injury. This evaluation process and the "percentages of loss"
of a body part were developed by The American Medical Association and published in a book, The Guides to the Evaluation
of Permanent Impairment.
INFORMED CONSENT AND QUESTIONNAIRE PACKET INSTRUCTIONS
For an FCE, an Informed Consent and Questionnaire Packet will be mailed to you. Please Read the Informed Consent, but Do
Not Sign It until you arrive for your FCE so we can witness your signature. But please complete the Pain and Disability Questionnaires before
you arrive for your FCE and bring the whole Informed Consent and Questionnaire Packet with you to your FCE.
Please DO NOT BRING CHILDREN with you for your FCE or PPD Evaluation. Children are Not Permitted in the evaluation area and there
is no area where they can be left unattended. If
you have children, please make arrangements for them and do not bring them with you for
your
FCE and PPD Evaluations.
Please TURN OFF CELL PHONES before you are taken back for your FCE or PPD Evaluation.
Cell Phones are very disruptive, so
please make sure that there are no calls that you have to take during your evaluations.
If an emergency arises and you have to cancel and reschedule your FCE or PPD Evaluation, PLEASE CALL US with As Much Advanced Notice
As Possible so we may schedule someone
else in your time slot. Thank You for This Courtesy. Please Call (478) 475-9393 between
1:00 pm
and 5:00 pm to cancel your appointment and to reschedule it for another date.
For a PPD, please complete and bring with you the Questionnaire Packet before you arrive for your PPD evaluation. There is No Informed
Consent for a PPD Evaluation.
If you have a Shoulder or Neck injury, wear a Short Sleeve or Sleeveless top so your Shoulder and Upper Arm can be examined.
If you have a Foot, Ankle or Lower Leg injury, please make sure your Shoes and Socks can be easliy removed for proper testing.
The permanent loss of function of a body part is based on many factors, including
the Diagnosis, Loss of Motion, Weakness, Type
of Surgical Procedure, Ligament Damage, Nerve Injury and other similar factors. Most PPD’s take less than one hour
to
complete and it's important to wear appropriate clothing to your evaluation.
If you have any questions regarding the proper clothing to wear to your PPD evaluation, please call our office for specific
instructions. Thank you.
INSTRUCTIONS REGARDING CHILDREN
One family member or one friend is welcome to observe your FCE, if you wish, and since FCE's may last a long time,
please come prepared to be in our center for a long time. And please call us if you have any problems related to your
FCE appointment.
